WIT 2003 Implementation Forms

 

WIT 2003 Participant:

Congratulations on completing the implementation of your WebQuest or curriculum web. You can use this form to tell us about your implementation and also to reflect on what went well and what did not work so well. This may help you to plan revisions to your project for the next time you implement it.

In order to prove implementation, you must also have at least 10 students who used your WebQuest or Curriculum Web submit the Student Evaluation Form. Two forms of the Student form are available: one for primary students and one for older students. Both are available here.

If you are a CPS teacher and would like to receive the remainder of your WIT 2003 stipend, you also need to attend at least one of the follow-up meetings planned by your homeroom mentors.

Once all three of these steps have been completed (you've submitted this form; 10 of your students have submitted an evaluation, and you've attended at least one follow-up meeting), then we will ask CPS to pay the remainder of your stipend.
